Two fouls by fatal accident in the North Sea – Beautiful weather

The PSA has identified two fouls after the accident in December where a wave hit the rig COSL Innovator in the North Sea. One person died and four were injured in the accident.

OIL AND GAS: the wave hit the device on the port side of the deck box provided and pressed into 17 windows, six on the lower deck and 11 on the middle deck, writes Authority in its report.

staterooms received extensive damage, and one person were killed and four others were slightly injured.

the investigation has proven two breaches of the rules:

  • COSL Innovator had no clearance of 1.5 meters between the bottom of the lower deck and highest crest. Covered was not designed to withstand horizontal wave loads.
  • Count system during patterning did not work satisfactorily.

COSL Drilling orders to do repairs before 1 October this year. A plan of work must be submitted by 13 May.
